Sickness Self Certificate
You only have to give your employer a fit note (sometimes called a sick note) if you’re off sick for more than 7 days in a row (including non-working days). For periods of less than 7 days you must self certify using this form Self Certificate Form.
Acute Kidney Injury Leaflet
If you are unwell and unable to drink properly, particularly if you are losing extra fluid through
vomiting or diarrhoea, then some medications can adversely affect your kidneys and so should be temporarily stopped whilst you are unwell. This information leaflet details what you should do in the event of sudden/acute illness.
