We can only prescribe ADHD medication under a ‘shared care agreement’ with an NHS provider
- if you do not attend you annual hospital review we will have to stop your prescription
- if you do not provide the information below we will have to stop your prescription
To enable us to safely continue to prescribe your medication we need a recent weight, blood pressure and pulse reading.
You can submit these here if you are able to monitor at home – if not please use ‘klinik’ to request a Health care assistant appointment to do this.
